Sonnweid in Wetzikon seeks a qualified Health or Care Professional (EFZ) to provide individualized, relationship-focused dementia care within a multidisciplinary team.
As part of a specialized competence center comprising 330 staff members, this position supports the daily care and well-being of 174 residents. The role operates within an institution recognized for its expertise in dementia care, requiring a professional who can manage nursing processes and foster meaningful connections with patients and their families.
Responsibilities
- Deliver individualized, relationship-oriented nursing and accompaniment to residents.
- Plan and implement the nursing process, including the design of living environments and daily activation activities.
- Act as a reference person for assigned residents, ensuring continuity of care.
- Support and counsel relatives, including participation in family meetings and case discussions.
- Collaborate interdisciplinary with team members, physicians, and external partners.
- Assume daily operational responsibility for assigned tasks.
- Maintain accurate nursing documentation and ensure clear information flow within the team.
Requirements
- Successfully completed vocational training as a Health Professional or Care Professional (EFZ).
