Assistant social (h/f)

Responsabilités

• Conduct social situation assessments, follow-up, and social support for patients, considering the psychiatric dimension.
• Coordinate the patient's social care in collaboration with center staff and external networks (CMS, CSR, SCTP, DGEJ, etc.).
• Conduct educational interventions and support in concrete steps (economic, administrative, professional, relational, legal, housing search, institutional placement).
• Act as the administrative reference point to prevent the loss of social benefits before an adult protection measure is established by the Justice of the Peace.

Profil recherché

• Bachelor HES in Social Work, social service orientation, or equivalent title.
• Professional experience allowing for complete autonomy in practice.
• Competence in assessing the social situation of patients with mental disorders.

Atouts

• Experience in psychiatry is an asset.
• Experience in management and team leadership is appreciated.
